Before I began to sew professionally, to keep my son's hands out of his diaper, I would sew snap tape into a pair of underwear into the waistband and at the bottom hem of a tshirt and snap them shut. You just have to sew straight around the snap tape. You can sew it by hand.

 I also used over sized tshirts and would join them at the crotch with bed sheet tie down things. Not sure what they are called, but they go under your sheets at the corners and lock onto the fabric.

My kids have a hard time using regular straws because they bite down too hard. I use rubber tubing used in fish aquariums, cut down to fit cups. Breast pump tubing also works.
